In accordance with New York State Health Department guidelines and Hoosic Valley’s reopening plan, we are required to have parents/guardians complete a periodic screening questionnaire for every student that will be coming to school for in-person learning. At this time, we will be requiring this questionnaire to be completed by 7:00 pm every Sunday night. Please click on the link below to complete the questionnaire.
