REQUIRED HEALTH PAPERWORK FOR INTERSCHOLASTIC ATHLETICS
Regulations of the Commissioner of Education, Part 135.4 (7)(h).
All students must have a valid "blue card" at the start of each sports season. Blue Cards are obtained by having a current physical on file with the school nurse, as well as submitting either the Interval Health History or the Athletic Health History form to the nurse. The school nurse will maintain student records upon submission of the following documents and deliver an updated blue card to each athlete's coach. Students must have satisfied the requirements of a blue card in order to participate on the first day of practice.
Components of the blue card include:

Interval Health History: For the student wishing to participate in athletics who has previously participated in the school’s interscholastic athletic program. This document can be submitted no more than 31 days prior to the first day of practice.

Comprehensive Physical Examination - required yearly and can be completed by a private MD or the school physician, Dr. Luke Nightingale.
Note: A "current physical" is defined as having been completed within one year of the month in which the sport starts. The summer is the ideal time to have physicals completed and submitted to the school nurse, as a physical on or after August 1st would satisfy the physical examination requirement for the entire school year. A Health History form must be completed prior to each sport season (e.g. a three-sport athlete must submit a Health History form three times - each one should be submitted within 31 days of the start of each respective sport season).
