Hoosic Valley competes in New York State's Section II, as a member of the WASAREN league. The Hoosic Valley Indians represent the district in 11 different sports.

ATHLETIC BLUE CARDS

 

  • A current physical is required - If the 12-month period for the physical expires during a sports season, participants may complete the season as long as an interval health history was conducted prior to the season. For example, a physical conducted August 1 would be valid through August 31 of the following year. If a student plays beyond August (ex. Football), the student may complete that sport season as long as an interval health history was conducted before the start of the season. Immediately following the last sanctioned tournament competition for that season, a new physical is required if the student is going to play another sport.
  • Interval Health History OR Athletic Health History (for 1st time participants) must be completed (dated no earlier than 30 days prior to the start of the sport season) and presented to the school nurse with a current physical for a Blue Card.
  • Current physicals and health history forms can be mailed or faxed to the High School (fax # 753-4151).
  • The nurse will issue blue cards directly to coaches after all paper work has been received.


INTERSHOLASTIC ATHLETIC HEALTH APPRAISALS - (Sports Physicals)

Regulations of the commissioner of Education, Part 135.4 (7)(h).

Prior to each sport season, school nursing personnel must review the student’s attendance records and either interview the student to update the health history or have the parent complete an interval health history in order for the 12 months physical examination to be valid.

All students must have a valid "blue card" at the start of each sports season. Blue Cards are obtained by submitting either the Interval Health history or Athletic Health History to the school nurse. The school nurse will review the students attendance records and health history and return an updated blue card to the student. Students must have a blue card to give to coaches the first day of practice.

The school physician, Dr. Nightingale, visits the Jr/Sr High School and the Elementary School one morning per month. If you would like to schedule a physical with the school physician, please call the respective health offices to make an appointment.
