Register New Students

Welcome to the Hoosic Valley School District, we are very excited that you and your family have moved into the District and look forward to meeting you soon. Our district has two school buildings, students in grades K-6 attend the Elementary School and students in grades 7-12 attend the Jr. Sr. High School. New students register in the building they will be attending. If families have students that will be attending both buildings they must register in both buildings.

Please check with Mrs. Vautrin or Mrs. Anderson prior to registration to verify office hours and schedule a time to review and complete all the necessary paperwork.

The following forms need to be completed as part of registration in the district.

Step 1: Housing Questionnaire - Complete this form first, if your are currently not living in permanent housing, proof of residency is not required.

Step 2: Enrollment form

In addition to the enrollment forms, we will need copies of the following documents:

Proof of Residency - Two forms are required (unless living in temporary housing)

Mortgage statement, Tax Bill, Contract of sale of house, Rent Receipt, Lease Agreement, Utility Bill (cable, fuel, electric, phone) Driver License, Voter Registration card. Additional documents may be used for proof of residency - please contact us for more information

Child’s birth certificate

If your child's birth certificate is not available we will accept a driver's license, state or government issued ID, school photo with date of birth, consulate ID card, hospital or health records, military dependent ID card, court order or other court documents, native american tribal documents or records from non-profit international aid agencies.

Custody or Guardianship papers – if applicable

In the absences of court documents please provide an affidavit stating that you are in a parental relationship to the child or have total and permanent custody and how that was obtained.

Step 3: Authorization to Release Student Records - including Special Education plan if applicable

Step 4: Additional information needed, you may complete and print forms prior to visiting the district or complete onsite
